M.S.A.D. #1 requires the following for participation on an interscholastic sports team:
- A pre-participation sports physical examination every other year between grades six and high school graduation (a pre-participation sports physical may be performed during the spring of a student’s fifth grade school year).
- The “B” and “C” forms are required yearly.
- Please note that if there has been a serious incident or accident the student would need a new physical or letter of clearance from his or her medical provider.
At this time the Health Services Office is preparing for the 2014-2015 school year. All prospective MSAD#1 athletes will need a new “B” & “C” form (enclosed) completed each year. It is also a requirement that your child have a current sports physical on file.
The procedure for obtaining a pre-participation sports physical has changed. In the past, the pre-participation sports physical has been offered and completed during the school day at MSAD#1 schools by a health care provider (doctor) from TAMC. The TAMC health care providers are no longer available to provide a sports physical in this manner. Therefore, you will need to arrange to have your child’s pre-participation sports physical done through their private health care provider.
Our school physician, Dr. Morningstar, is a member of the Aroostook Pediatrics team. If your child is not a member of the Aroostook Pediatrics practice, and you do not have a primary health care provider, Aroostook Pediatrics has agreed to perform a sports physical for your child. They have asked that you provide an immunization or shot record (the Health Services Office can help you with this) at the time of the physical. Please call Aroostook Pediatrics at 764-KIDS to schedule an appointment at their office on North Street. Once the physical is completed, Aroostook Pediatrics will fax the information to the Health Services Office at 768-3085.
Should your child’s current private health care provider perform the sports physical, please ask that they fax a statement clearing the student for athletic participation to the Health Services Office at 768-3085.
Insurance companies frequently have their own requirements concerning coverage for physicals. If this is so for your child, please contact the Health Services Office. If you are not sure if your child needs a physical, please call the Health Services Office and we will be able to tell you if your child is due for a new sports physical or not. All fifth grade students will need a physical in order to try out for sports at Presque Isle Middle School in June.
